I was wondering if it is feasible that the so-called "MADI" optical interface can instead be used for either ADAT or S/PDIF alone?

HI,
No it's a completely different connection and circuit.
You need the RME ADI-648 MADI to ADAT or RME ADI 6432 MADI AES format converters.

Can you tell if the Headphone output can be used as a line level output to say feed an amplifier in a balanced or unbalanced fashion?

HI,
On the MADI card?
Yes that will work fine. It is hardwired to outputs 63/64 but with total mix you can send any other channel to it also.
