If a 12V DC power supply is in operation and then the 12V plug is plugged into a device, this leads to a considerably higher inrush current in the 12V device than if everything is connected and the mains switch is turned on. If this describes your situation and the power supply is OK, then the old ADI-2 has a technical problem.
A - probably only short-term - solution:
Get a time relay for mains operation and delay (> 1 second) the switching on of the 12V line to the ADI-2 with the relay.
Problem of the ADI-2 could have led to overload and defect of the power supply unit.
Are all analogue outputs (also headphones) of the ADI-2 still faultless?
